Wanamassa First Aid Squad is committed to protecting our patients' personal health information and privacy in compliance with the federal Health Insurance Portability and Accountability Act of 1996 (HIPAA). The following requirements must be met before the release of medical records.

• Wanamassa First Aid Squad, Inc. requires a subpoena and/or an Authorization for release/disclosure of health information signed by the patient or his/her guardian.
• All requests must be made using a Patient Consent & Authorization for Release of Protected Health Information form.
• All requests must include date, incident location, and a contact telephone number for verification.
• Fees
  • Search Fee: $20.00 per request
  • Certification Fee: $10.00 per request
  • Paper or Electronic Documents: $1.00 per page
    • Cap for each patient request is $50.00 inclusive of the above fees.
• Checks made payable to Wanamassa First Aid Squad, Inc.
• Records will not be released without payment.
• Release of medical records will not occur until these requirements are met and mailed to:
